Applying for Housing
If you are interested in living in the residence hall at CCS, you must fill out an application, complete the Medical History and Health Information forms and sign the Resident Acknowledgment form.

All students living on campus must have health insurance and a local doctor who has agreed to accept them as a patient. See the following section for suggestions on finding a local doctor.
Submit your completed forms, along with a photocopy of the back and front of your health insurance card, the $200 Housing Application Deposit (refundable until May 1), and the initial $350 housing payment to:

It is the responsibility of the student to inform CCS of any changes in their health not recorded on their Medical History form.
Finding a Local Doctor
Suggestions for finding a doctor in the Detroit area:
- Contact your health insurance company for a list of doctors in the Detroit area.
- Call a physician referral service, which will match you with a doctor by specialty, location, and insurance coverage. Referral services, such as St. John Health System, will even give you background information on the doctor. Following are some Detroit area physician referral services:

St. John Health Systems: 888.757.5463 - Check the internet at Yahoo Yellow Pages or Yellowpages.com. Select a doctor. Call and make sure the physician is accepting new patients, and that they accept your type of health insurance.
- If you know anyone in the area, ask if they have a doctor they would recommend. Consider asking other students as well.
